Veterinary authorisation is required prior to treatment. Please download and complete the veterinary authorisation form. The form must be signed by your vet before the first appointment.

Please also ensure that you have read and signed the Guidelines / Terms and Conditions. 

Both forms must be brought along on the day of your first appointment.
